Dr. Bhatia’s new patients ordinarily fill out the following three forms: a patient registration form, a brief gynecological history, and a records release (in the event of a transfer from another physician). For your convenience, we have included these forms below. Please feel free to review the form as preparation for your appointment or complete a hard copy and bring it in to your first appointment.
